Clinical Referral & Care Coordinator at Providence Health and Services summary:

The Clinical Referral & Care Coordinator is a key member of a multidisciplinary healthcare team, managing specialty referrals, coordinating patient services, and facilitating access to community resources. This role supports patients through education and seamless transitions of care, particularly assisting those who struggle to navigate the healthcare system. The position requires healthcare experience and focuses on outpatient care coordination to improve patient outcomes.

Description

The Clinical Care Coordinator participates as an integral member of the multidisciplinary care team at PMGA. Key roles are coordination of services for panel patients, focusing on those who face particular difficulty with navigating the healthcare system, management of specialty referrals and supports patients in self-care and identifying relevant community resources. The Clinical Care Coordinator collaborates with and outreaches to the outside service providers to provide seamless transitions of care, and provides patient education while assisting with access to applicable resources.

Required Qualifications:

  • Associate's Degree in Healthcare Related, or in lieu of Associate's degree: 2 years of experience in an outpatient setting OR related coursework/training and 1 year of experience in an outpatient setting

Preferred Qualifications:

  • Bachelor’s Degree in Healthcare Related
  • 2 years care coordination experience.
  • Experience in an outpatient setting.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
