Job Title: Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

Employment Type: Full-Time
Salary Range: $80,000 - $110,000 annually

Position Overview:
We are seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our team. This full-time position will involve providing high-quality care to patients at both Encompass Health Rehabilitation Hospital and Shah Medical Clinic in Sioux Falls. The ideal candidate will have a strong background in rehabilitation and internal medicine, with a commitment to patient-centered care.
Key Responsibilities:
• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Collaborate with multidisciplinary teams, including physicians, therapists, and nursing staff, to coordinate patient care.
• Provide ongoing medical management and adjust treatment plans as necessary.
• Educate patients and their families about health conditions, treatment plans, and preventive care.
• Participate in quality improvement initiatives and contribute to the development of best practices.

Encompass Health Rehabilitation Hospital of Sioux Falls is a 40-bed, freestanding inpatient rehabilitation hospital dedicated to helping patients regain independence after life-altering illnesses or injuries. Our multidisciplinary team provides compassionate care through specialized programs, including neurological, orthopedic, stroke, and spinal cord injury rehabilitation. We emphasize personalized treatment plans to meet each patient’s unique goals.
Shah Medical Clinic, led by Dr. Syed A. Shah, offers comprehensive internal medicine services with a focus on individualized patient care. With over 30 years of experience in South Dakota, Dr. Shah and his team are committed to improving patients’ health and well-being.
Qualifications:
• Current licensure as a Nurse Practitioner or Physician Assistant in South Dakota.
• Certification from an accredited NP or PA program.
• Minimum of 2 years of experience in rehabilitation, internal medicine, or a related field preferred.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a multidisciplinary team environment.
